Role Overview: The Care Coordination RN Sr. assesses transitional care needs, coordinates care across the continuum, and collaborates with patients, families, and care teams to ensure optimal patient outcomes during acute hospital stays and post-discharge planning.


Key Responsibilities:

  • Conduct comprehensive psychosocial and clinical assessments, addressing social determinants and resource needs.
  • Facilitate communication among physicians, care teams, and patients to promote care progression and discharge planning.
  • Identify and resolve barriers to discharge, ensuring timely and efficient patient transitions.
  • Manage all aspects of discharge planning, including developing individualized plans and coordinating post-acute referrals.
  • Document assessments, care plans, and interactions accurately in the medical record.
  • Mentor and precept new staff and interns, supporting professional development.
  • Support department goals through ongoing competency assessments and adherence to policies.

Qualifications & Skills:

  • Minimum 3 years of acute care nursing experience and 2 years in hospital-based care coordination.
  • Valid RN license and BLS certification required.
  • Strong communication, assessment, organizational, and problem-solving skills.
  • Knowledge of healthcare regulations and community resources.
  • Ability to work collaboratively in a fast-paced, multidisciplinary environment.
