Role Overview:
The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary setting. They are accountable for program development, quality improvement, problem-solving, and enhancing productivity within a collaborative team environment.
Qualifications & Skills:
- Minimum education: High School Diploma (required) or advanced degrees preferred.
- Doctorate of Physical Therapy (DPT) required after certain date.
- Experience: Emphasis on years and type of experience preferred but not mandatory.
- Licensure & Certifications: Valid Texas Driver’s License, licensed Physical Therapist, Basic Life Support (BLS), Auto Liability Insurance, reliable transportation, and other relevant licensures as specified.
Key Responsibilities:
- Perform assessments, develop treatment plans, and deliver therapeutic interventions.
- Contribute to program development and quality improvement initiatives.
- Work within an interdisciplinary team to ensure optimal patient care.
Required Skills & Certifications:
- Licensed Physical Therapist with required licensure and certifications.
- Valid Driver’s License, BLS certification, auto insurance, and dependable transportation.
- Must obtain certain certifications within ninety days of hire.
