Role: LMSW Case Manager within the Community & Social Services team, focusing on Healthcare & Medical Services.
Responsibilities:
- Conduct comprehensive psychosocial assessments.
- Develop person-centered care plans tailored to patient needs.
- Coordinate medical, behavioral health, and community resources for diverse populations.
- Collaborate with interdisciplinary teams to reduce care barriers and support chronic disease management.
- Promote patient self-advocacy and engagement.
- Utilize data to inform practice and ensure continuous quality improvement.
- Maintain clear documentation within EMR systems to support service delivery and outcomes.
Qualifications & Skills: Relevant social work licensure (LMSW), strong assessment and care planning skills, ability to work collaboratively across teams, excellent documentation abilities, and a focus on quality improvement.
