Seeking a Speech Language Pathologist for our Home Care team to provide personalized assessment and treatment in patients’ homes, enhancing communication, cognition, and swallowing skills.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based care plans, educating patients and caregivers, documenting progress, and collaborating with a multidisciplinary team.
Qualifications: Master’s degree in Speech-Language Pathology, CCC or CFY, NYS license, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
