The multi-disciplinary team includes a leader, clinicians, care managers, and peer specialists, utilizing the evidence-based Critical Time Intervention model. The role involves community outreach, assisting participants with housing transition, enhancing daily living skills, offering supportive counseling, and advocating against discrimination and healthcare disparities. Core principles include member choice, harm reduction, non-coercion, flexibility, and a person-centered approach. Post-housing support continues for months to ensure stability and independence. This field-based position requires travel across three counties, periodic on-call duties, and flexible hours. Qualifications include a licensed master’s degree in social work, counseling, nursing, or psychology; MSW candidates with plans for licensure are considered.
