Role Overview: A licensed Physical Therapist assesses, plans, and implements rehabilitative programs to improve mobility, reduce pain, increase strength, and prevent deformities in patients with illness or injury. Services are delivered across the continuum, emphasizing quality, cost-effectiveness, and patient-centered care, while ensuring compliance with regulatory standards.
Key Responsibilities:
- Treatment Planning & Care: Develops, implements, and adjusts treatment plans in collaboration with patients and families; provides education, discharge instructions, follow-up, and referrals.
- Patient Assessment: Establishes and evaluates treatment programs, goals, and discharge plans in partnership with physicians and families.
- Documentation: Maintains comprehensive records of evaluations, treatment sessions, and discharge information per departmental protocols.
